Family Care Doctor or Pediatrician- There are numerous doctors in the area who will be happy to follow the care of your child.  This doctor is typically the primary person to conduct an annual health screening (physical, well-check).  It is important to complete an annual health screening since doctors are trained to not only look at the physical health of the body but to also screen that your child is meeting the typical developmental milestones for his/her age. We require an annual well-check for all students who are in our program.  Please contact your insurance provider or Pediatric Services (see link below)
Annual Physical Form
This form must be completed annually.  You will receive a reminded when it is time to update the physical which is on file.

Shots for Tots
Vaccinations are an important way to keep your child healthy and safe.  Shots for Tots is a great program for families to obtain necessary vaccinations.  Please click the link to the left to find locations.

Help Me Grow
Help Me Grow is Ohio’s evidenced-based parent support program that encourages early prenatal and well-baby care, as well as parenting education to promote the comprehensive health and development of children. Help Me Grow System includes Central Intake, Help Me Grow Home Visiting and Help me Grow Early Intervention.
